CHAP was founded nearly 30 years ago with a mission to help individuals and families overcome the health and social barriers that prevent long‑term stability. What began as a small grassroots effort has grown into a nationally recognized Community Health Worker program that strengthens care coordination across North Central Ohio. Today, CHAP supports organizations that employ Community Health Workers in Richland, Crawford, Marion, Knox, and Huron counties, helping thousands of residents access medical care, transportation, housing support, insurance, and essential resources. Through trusted relationships, strong partnerships, and a commitment to measurable outcomes, CHAP continues to serve as a vital connector for families working toward healthier, more stable lives.


CHAP is committed to supporting families in Crawford County by helping provide essential items that many households struggle to afford. Through our partnership with the local Goodwill, we see firsthand how diapers, wipes, hygiene supplies, transportation assistance, and other basic necessities can immediately reduce stress and improve stability for participants. Every dollar raised will directly supply these critical items, ensuring families in Crawford County have the practical support they need to stay healthy, safe, and moving forward.
